Delegated Staff:

All Directors, Managers, Staff appointed by Management and Creditors Clerk.

 

Wesbank have standard forms that must be used when a petrol card is damaged, lost, stolen, vehicle has to be disposed of or new vehicle added to the fleet.
 

The following  forms must be used:

These  forms must be completed by the Department Managers or appointed staff and sent to the Creditors Clerk for Authorisation.
 

The completed form is faxed Wesbank by the Creditors Clerk.
 

The Lost / Stolen Form  “ Form must be completed by the department manager or appointed staff and faxed directly to Wesbank fraud dept. Send the copy to Creditors Clerk for filing.
 

The Creditors Clerk files the forms in the Petrol  file.
